You agree to use this website lawfully and not to attempt to disrupt, reverse engineer, or misuse any part of it. Content on this site — text, design, and imagery — is owned by Nephron MD and may not be copied or republished without written permission.

Nephron MD is a concierge nephrology practice. Care is offered through a membership relationship and does not begin until membership is formally accepted by both parties. Submitting a contact form does not establish a physician–patient relationship and does not guarantee acceptance into the practice.

We intentionally cap our panel at 25 patients between Dr. Anand Prakash and Dr. Deepak Chandramohan. We may decline membership when accepting a new member would compromise care for existing members or when our practice is not the right fit for your needs.

Membership fees are agreed to in writing prior to enrollment. Membership is paid directly to the practice and is separate from any insurance coverage you may carry for outside labs, imaging, hospital care, prescriptions, or other services. Refund and cancellation terms are detailed in your membership agreement.

Members receive direct physician access, scheduled and unscheduled visits, secure messaging, virtual visits, coordination with outside providers, and a written care plan. Specific scope is defined in your membership agreement.

To the fullest extent permitted by law, our liability arising from use of this website is limited. Liability for clinical care is governed by the laws applicable to medical practice and by your individual membership agreement.

These terms are governed by the laws of the State of Texas. Any dispute arising from use of this website will be resolved in the appropriate courts located in Collin County, Texas, unless otherwise required by law.

We may update these terms from time to time. The "last updated" date at the top of this page reflects the most recent change. Continued use of this website after updates constitutes acceptance of the new terms.
