Introduction
NephronMD Kidney Care was created because kidney patients deserve better. In a typical medical system, appointments are short, waiting rooms are long, and patients often leave with more questions than answers. We built NephronMD to fix that. We are a concierge nephrology practice, which means we limit the number of patients we see so that every single person gets the time, attention, and care they truly deserve.

Kidney care should never feel like a conveyor belt. My commitment is to give every patient the time, expertise, and continuity that complex kidney and metabolic disease demand — and to be a partner you can actually reach when it matters.
Education
Dr. Prakash earned both an M.D. and an M.S., followed by rigorous postgraduate training in Internal Medicine and Nephrology in the United States.

I am committed to helping patients with kidney disease navigate their health with clarity, dignity, and confidence. By combining evidence-based treatments with compassion and open communication, I strive to support each patient in achieving a healthier and more fulfilling life despite the challenges of chronic illness.
Education
Dr. Chandramohan began his medical education in India, where he completed medical school before moving to the United States. He completed his Internal Medicine residency at Coney Island Hospital, Brooklyn, NY, followed by a Nephrology Fellowship at the University of Alabama at Birmingham.
